This role focuses on residential referrals, meet-and-greets, coordination with client care teams, and supporting client financial and personal needs prior to program enrollment. This role advocates for clients, ensures person-centered care, and supports program growth by fostering positive relationships with clients, families, and community partners. Key Responsibilities: Manage all Supported Living Services residential referrals and coordinate timely placements. Facilitate meet-and-greet appointments between prospective clients and care teams. Collaborate with Representative Payees to establish client financial support prior to enrollment. Collaborate with Representative Payees to find housing for new clients. Establish and maintain relationships with non-profit organizations that support housing for IDD clients. Participate in client transition meetings to ensure smooth onboarding into SLS programs. Advocate for clients’ needs, preferences, and goals to ensure person-centered, appropriate placements. Support program growth by identifying opportunities for new client enrollment and enhancing service delivery. Maintain accurate documentation and reporting regarding referrals, transitions, and client-specific needs. Ensure compliance with all Washington State SLS regulations and organizational standards. Submit referrals within 10 days of receiving them. Track referrals on the referral tracker, submit them timely to the Residential Director to review and maintain data. Work with the Client Care Team to make sure that potential client placements are person-centered Work with the Area Directors to fill vacancies in programs and maintain 90% census at all times. Qualifications: Experience in Supported Living Services, developmental disabilities services, case management, or related fields. Knowledge of person-centered planning and SLS best practices. Strong communication, relationship-building, and advocacy skills. Ability to coordinate with multiple stakeholders, including clients, families, care teams, and financial representatives. Excellent organizational skills, attention to detail, and the ability to manage multiple priorities. Travel within Washington State may be required for meet-and-greets, site visits, and community outreach. Skills Demonstrate strong interpersonal communication skills, responding to requests and providing clear feedback within established timelines. Maintain a solution-focused approach to program management and concerns. Exhibit strong organizational skills to manage deadlines and address emergent needs effectively. Stay current with technology, demonstrating proficiency in Google Suite and Microsoft applications. Commit to person-centered solutions through collaboration with clients, guardians, and staff. Build relationships and network with community members, DDA, families, and organizations supporting our clients. Position Type and Expected Hours of Work Full-time, Salary Typical hours: Monday through Friday, 8:30a-5:30 pm, with frequent after-hours attention required. Travel Travel between offices, programs, and meetings averages 80%. Personal vehicle use; mileage reimbursed. Out-of-state travel is typically not necessary. Required Education and Experience High School Diploma 2 years’ experience working with clients with disabilities 3 years supervisory experience Preferred Education and Experience Bachelor’s Degree in Human Services or related field 3 years working with people with disabilities 5 years managing teams Training Expectations Certified in Crisis Prevention Institute, Right Response, or similar training Certified in CPR/First Aid Ongoing Eligibility Qualifications Pass background check screening upon hire and every two years thereafter, or as requested. Maintain active NAR, CPR/First Aid, Nurse Delegation, and other required certifications. Complete a minimum of 12 hours of continuing education annually.
